Club Membership Agreement

This Membership Agreement (“Agreement”) is entered into between Blu Monarch (“Practice”) and the undersigned patient (“Member”).

By enrolling in Club X or Club Evolus, the Member agrees to the following terms and conditions:

Membership Overview

Club X and Club Evolus are monthly membership programs designed to provide consistent neurotoxin maintenance, preferred pricing, and exclusive member benefits.

  • Monthly Membership Fee: $49 per month
  • Minimum Treatment Requirement: Member must receive a minimum of 30 units per treatment visit
  • Included Benefit: 20 units of neurotoxin every 90 days
  • Club X: Xeomin
  • Club Evolus: Jeuveau
  • Membership provides a $320 savings compared to standard pricing

Member-Only Benefits While Enrolled

  • 15% off retail products
  • 10% off esthetic and IV therapy treatments
  • Special member pricing on Tox Booster treatments (available every 90 days)
  • Preferred neurotoxin pricing on additional units beyond the included amount

Membership fees are billed monthly and are not applied toward services or products outside of the listed membership benefits.

Treatment Schedule

  • Included units must be used during a scheduled appointment every 90 days.
  • Units do not roll over and must be used within the designated treatment window.
  • Appointments are subject to provider availability and must be scheduled in advance.

Additional Units

  • Membership requires a minimum of 30 units per treatment visit.
  • The included 20 units are applied toward this minimum.
  • Any additional units required beyond the included 20 units will be billed at the member-preferred neurotoxin rate.
  • Placement and dosing are determined by the provider based on facial anatomy, movement, and aesthetic goals.

Minimum Commitment & Cancellation Policy

  • Membership requires a minimum commitment of 90 days.
  • Cancellation before the first 90 days are complete will result in a $100 early cancellation fee.
  • After the initial 90-day period, the Member may cancel the membership at any time without penalty.
  • Cancellation requests must be submitted in writing or through the Practice’s designated communication platform.

Missed or Late Appointments

  • Failure to schedule or attend a treatment within the 90-day window does not result in a credit or refund.
  • Membership fees are not refunded for missed, delayed, or unused treatments.

Medical Disclaimer

  • Neurotoxin treatments are medical procedures.
  • Treatment eligibility is determined by a licensed provider.
  • The Practice reserves the right to defer or decline treatment if medically contraindicated.
  • Results may vary, and no specific outcome is guaranteed.

Membership Modifications

  • Membership pricing, benefits, or terms may be updated with prior notice.
  • The Practice reserves the right to discontinue the membership program at its discretion, with reasonable notice provided to active members.

Non-Transferability

  • Membership benefits are non-transferable and may only be used by the enrolled Member
  • Membership cannot be shared, gifted, or applied to another individual.

Acknowledgment & Consent

By signing below, the Member acknowledges that they have read, understand, and agree to the terms of this Membership Agreement and consent to participation in the selected Club Membership.
